Mentorship Agreement

Students observe. They do not practice.

Students shadow and learn. No clinical tasks, no prescribing, no unsupervised patient contact.

Provide meaningful exposure — not just presence.

Rotations are 2–4 weeks. Include rotation at a hospital or in office settings. Provide mentoring and teach basic concepts of your specialty.

Write the Letter of Recommendation.

If the student completes the rotation satisfactorily and requests a letter, you provide it within 30 days.

Cover HIPAA and expectations on day one.

On the first day, cover: patient privacy (no photos, no sharing, no social media), dress code, the observation-only role, rotation schedule, and how to reach you.

Notify ShadovMentor if something goes wrong.

If a conduct or safety issue arises, notify admin@shadovmentor.com within 48 hours. Either party may end the rotation at any time with reasonable notice.

ShadovMentor is an introduction platform. We do not supervise clinical care or assume liability for events in your setting.
Your digital signature is legally equivalent to a handwritten signature and will be recorded with the date and time.

Student Registration Agreement

Section A — Student Information
Your registration information submitted at shadovmentor.com is on file. This agreement applies to the student who completed the online registration form.
Section B — Academic Standing Attestation
1.I am currently enrolled in or have graduated from an accredited educational institution.
2.I have not been dismissed, suspended, or placed on academic probation for conduct violations.
3.I have no criminal convictions.
Section C — HIPAA & Patient Privacy
⚠ HIPAA NOTICE — PATIENT PRIVACY IS A LEGAL OBLIGATION

I will be present in a HIPAA-covered clinical environment. The following are non-negotiable:

• I will not photograph, record, or share any patient information in any format or on any platform.
• I will not share patient information with anyone outside the clinical team, including family and friends.
• I will not discuss patient cases outside the clinical setting.
• Violations of patient privacy will result in immediate removal from ShadovMentor.
If you are unable to submit, email admin@shadovmentor.com with any questions and describe your scenario.

High School Agreement & Parental Consent

For high school students — parent or guardian signature required

Section A & B — Student & Parent Information
Your registration information is on file. This agreement applies to the student who completed the online registration form and the parent or guardian who provided consent.
Section C — Age & Eligibility
1.The student is currently enrolled in high school.
2.The student is 16 years of age or older.
3.The student is in good academic standing with no active disciplinary actions.
Section D — HIPAA & Patient Privacy
⚠ HIPAA NOTICE — PATIENT PRIVACY IS A LEGAL OBLIGATION

Even in a high school shadowing experience, HIPAA applies fully. Patients have a legal right to privacy. A photo, a text message, or a social media post about a patient can result in serious legal consequences — regardless of age.

The student and parent/guardian both acknowledge the following:

• The student will be present in a HIPAA-covered clinical environment.
• The student will not photograph, record, or share any patient information or images in any form — including text messages and all social media platforms.
• The student will not discuss patient cases with friends, family, or on any platform outside the clinical setting.
• The student understands that HIPAA violations have serious legal consequences and will result in immediate removal from the rotation.

The supervising physician will review HIPAA obligations with the student on the first day of the rotation.

Why ShadovMentor exists

The student

A student with the drive, the scores, and a clear sense of what they wanted — but no US clinical experience and no way to get it. Every hospital said the same thing: apply through our GME office. Every GME office said the same thing: you need a preceptor first. No way in.

A practicing mentor who had trained alongside students from all backgrounds and knew exactly what they needed — not a lecture, but time in clinic, seeing real patients, learning how medicine is actually practiced. He wanted to give that. He just had no way to find the right student.

The demand for rotations, shadowing, and mentorship is enormous. What surprised me most is how universal that need is — physicians, NPs, PAs, nurses, pharmacists, and dentists all require hands-on clinical experience to move forward in their careers. ShadovMentor charges a flat platform fee per rotation week. Pricing is unified for all students — US and international. Mentors are paid directly through the platform after each rotation is confirmed complete.
